Just about all the main coincidence investigations--Texas urban, Piper Alpha, the Phillips sixty six explosion, Feyzin, Mexico City--show human mistakes because the valuable reason, both in layout, operations, upkeep, or the administration of protection. This e-book presents functional suggestion that could considerably decrease human errors in any respect degrees. In 8 chapters--packed with case reviews and examples of straightforward and complex thoughts for brand new and current systems--the e-book demanding situations the belief that human mistakes is "unavoidable." as a substitute, it indicates a structures viewpoint. This view sees blunders as a result of a mismatch among human services and calls for and irrelevant organizational tradition. This makes blunders a potential issue and, hence, avoidable.

Chemical Pretreatment of Nuclear Waste for Disposal

Chemical pretreatment of nuclear wastes refers back to the series of separations techniques used to partition such wastes right into a small quantity of high-level waste for deep geologic disposal and a bigger quantity of low-level waste for disposal in a near-surface facility. Pretreatment of nuclear wastes now saved at numerous U.

Gas leaked into the firebox during the repair period and was not removed. A severe explosion occurred during the attempt to light of. 2 Low water level had shut down a boiler. Flameout occurred on two attempts to refire the boiler. On the third attempt, a violent explosion occurred. The worker had not purged the firebox between each attempt to fire the boiler and this resulted in the accumulation of fuel-air mixture which exploded on the third attempt to ignite the pilot. 3 A boiler house enclosed eight large boilers attended by two men.

PURPOSE OF THE CHAPTER The purpose of this chapter is to provide a comprehensive overview of the main approaches that have been applied to analyze, predict, and reduce human error in industrial systems. The practical application of specific techniques to achieve these goals must be built upon an understanding of the theory that led to the development of these techniques. Just as it would be inadvisable for an engineer to attempt to design a venting system without an underlying knowledge of the behavior of chemical reactions, it is recommended that the user of human factors techniques becomes acquainted with their underlying rationale.

This is followed by an introduction to each of the approaches, which are then described in more detail in subsequent sections. 2. CONCEPTS OF HUMAN ERROR A single, all-embracing definition of human error is difficult to achieve. For the engineer, the worker in a system such as a chemical process plant may be perceived as being there to perform a set of tasks to achieve specific operational objectives. There is therefore relatively little interest in the underlying mechanisms of failure. For the human reliability specialist, however, who is attempting to predict and optimize human performance, the underlying organizational and psychological causes of errors are of considerable importance.
