CONCEPTS OF HUMAN ERROR A single, all-embracing definition of human error is difficult to achieve. For the engineer, the worker in a system such as a chemical process plant may be perceived as being there to perform a set of tasks to achieve specific operational objectives. There is therefore relatively little interest in the underlying mechanisms of failure. For the human reliability specialist, however, who is attempting to predict and optimize human performance, the underlying organizational and psychological causes of errors are of considerable importance.
